Background Investigation Information

 

DO NOT submit your Personal History Questionnaire (PHQ) until instructed to do so.  The PHQ does NOT substitute for an employment application.

  1. Complete the Personal History Questionnaire (PHQ) in one of the following formats:
    •  ***Microsoft Word  /  Adobe PDF
      • ***The Microsoft Word version is formatted to be typed on directly.  If completing the PHQ by hand or on a typewriter, please use black ink.
      • Click here for instructions for completing the PHQ.
  2. Complete both of the following forms and ATTACH them to the back of the PHQ.
    • Form 4506-T (Request for Transcript of Tax Return)
    • Form 180 (Request Pertaining to Military Records) will be available soon.

Candidates for employment with the City of Milwaukee Police Department must pass a thorough background investigation. After completing the written examination, you will be required to attend a Background Investigation Briefing presented by the Background Investigation Section of the Milwaukee Police Department. Once you have been notified of the date and time of the Briefing, you should review and print a copy of the Personal History Questionnaire (PHQ). The PHQ is the first step in the Background Investigation process.
