Background Investigation Information

DO NOT submit a Personal History Questionnaire (PHQ) unless instructed to do so. 

 

1. Complete the Personal History Questionnaire (PHQ) in one of the following formats:
   • Microsoft Word:  This version is formatted to be typed on directly.  Click here to download the online application.  Be sure to scroll to the top of the document for the instructions on Page 1 and 2!   Or, click here for the instructions for completing the PHQ.
   • Adobe PDF (click here):   This version must be completed by hand, or on a typewriter.  Please use only black ink.  Be sure to read the instructions on  Page 1 and 2.
2. Complete both of the following forms and ATTACH them to the back of the PHQ.  Click on the form number  to download and print.
   • Form 4506-T (Request for Transcript of Tax Return)
   • Form 180 (Request Pertaining to Military Records)
