Milwaukee County Wellness Initiative

Milwaukee County is pleased to announce a new wellness initiative, administered by APS Healthcare, available to all eligible employees effective March 25th, 2008. 

 

 

Active employees eligible for a Milwaukee County health plan (and their spouse, if covered by a Milwaukee County active-employee health plan) will have access to an array of services including health risk assessments, on-line self-help tools, a free NurseLine and wellness support programs for stress management, nutrition, tobacco cessation, and weight management.

 

This voluntary program is also designed to assist individuals in managing chronic health conditions and the impact they have on daily life. Utilizing a staff of registered nurses, dieticians, physical and respiratory therapists, certified diabetes educators, behavioral health therapists and physicians, participants can receive support, guidance and education on a condition and how to begin to control it.  Working with participants and their doctor, an APS Health Coach will help develop a personalized care plan to meet a participant's specific needs. Some of the Health Management programs available are:

 

* Diabetes

* High Cholesterol, Triglycerides

* Depression

* Coronary Artery Disease (CAD)

* Congestive Heart Failure (CHF)

* Hypertension (High Blood Pressure)

* Low Back Pain

* Chronic Obstructive Pulmonary Disease (COPD)

Select the option to be connected to a Health Coach who will help address concerns and develop a plan of action.  The free NurseLine is also at this number. APS may contact participants directly to describe the benefits of the program in greater detail.

 

Milwaukee County is excited about the services these programs offer our members and the opportunities they present to impact the quality of life. Employees are encouraged to take advantage of this new benefit at no cost, and completely voluntarily. In addition, Milwaukee County is offering a $100 incentive for eligible participants for completing the health risk appraisal with biometrics.
