Public Health Nurses from Milwaukee Health Department provide intensive medical case management to HIV positive women in Milwaukee, who are at high risk of not receiving medical care due to poverty, substance abuse and mental health issues. Funding is provided by the Community Development Grant Administration for a 12-month period. Community based organizations, HIV/AIDS service organizations, health care corporations, and educational institutions are eligible to apply for funds.
