What are the MHD's 4 Home Visitation Programs?

Core Team Public Health Nursing

Core Team offers short-term home visits city-wide to at risk parents and their newborns.  This program also serves as a safety net for high-risk infants under one who are in need of health assessments, health advocacy, referral and follow-up.  Core team also provides health services through our Walk-In and scheduled Family Health Clinics.

Empowering Families of Milwaukee

Empowering Families in Milwaukee (EFM) uses evidence-based models and partners with the community to provide frequent and long-term home visits to families. The focus of EFM is pregnant women and their children who reside in zip codes: 53204, 53205, 53206, 53208, 53210, 53212, 53218 and 53233.

Nurse-Family Partnership           

Nurse-Family Partnership (NFP) is an evidence-based nurse home visiting program for first-time pregnant teens and women. The nurses provide frequent and long-term home visits to families from early in their pregnancy until the child’s second birthday. The focus of the program is to improve pregnancy outcomes for the woman, to improve the child’s health and development, and to improve the economic self-sufficiency of the family

Parents Nurturing and Caring for their Children  

Parents Nurturing and Caring for their children is a team of nurses providing Prenatal care Coordination (PNCC).  PNCC is a Medicaid benefit that helps pregnant women get the support and services they need to have a healthy baby.  A public health nurse provides home visits during the pregnancy and until the baby turns two months old.
