Shared Vision of Milwaukee's Public Health Partners: "Milwaukee is a model community with healthy, safe, hopeful, and empowered residents." The City of Milwaukee Health Department (MHD) has initiated a citywide Community Health Assessment. This initiative is the most comprehensive health assessment ever undertaken by the City of Milwaukee, and will be used to help formulate our community's future public health goals and outcomes. MHD has chosen the health assessment model Mobilizing for Action through Planning and Partnerships (MAPP), developed by the National Association of City and County Health Officials and the Centers for Disease Control and Prevention. This process is a collaborative effort by the City of Milwaukee Health Department and a steering committee composed of leaders and representatives from a broad array of city departments, health systems, and community organizations. 2008 Steering Committee Meetings All meetings take place at the United Way of Greater Milwaukee. MAPP StructureThe Steering Committee includes two subcommittees, the Community Health Status Subcommittee and the Community Themes and Strengths Subcommittee. Each subcommitee conducted different assessments. The Subcommittee information is included below. Community Health Status Subcommittee Community Themes and Strengths Subcommittee MAPP ProgressThe Steering Committee has completed all four assessments and is working on the Strategic Issues and Planning Phase. This phase determines key issues and creates plans to address them. Learn more about this process at NACCHO's Phase Four Strategic Issues site. Update: Before the next Steering Committee meeting in September, we will be conducting community group discussions so we may receive more input on the health concerns of Milwaukee's citizens.
