What do I need to apply?
•A form of Identification with a Photograph.
Driver's License or DMV ID card or Passport
•Proof of Identity and legal Name.
Photo ID, Birth Certificate, Social Security Card or Alien Registration Card
•Proof of Milwaukee County Residency for 180 days (6 months).
Utility Bills or Current Lease
•Proof of Income.
Award Letters, Pay Stubs, or Taxes
•Verifiable Social Security Number
Card or SSN verification print out from the SSA office
•A check or money order for $50 made payable to GAMP. Fees will be waived for the months of November and December.
